All request forms require uNID and password authentication.
Please use the following forms to ask IT governance to:
- Add a service
- Retire a service
- Investigate a problem/consider a suggestion
- Evaluate a business case. Please explain in detail the research and rationalization for adding an IT service or product that has significant impact on the university’s network or users.
These forms are not intended to be used for help desk services, such as password resets, or requesting minor changes to existing services. They are meant to provide input into broader IT issues at the U.
