Tax season information

Note: Paper W-2 forms will no longer be delivered to university departments. This change is due to university efficiency improvements and the risk of access to personally identifiable information. In January 2019, W-2 forms will be received electronically via Campus Information Services (CIS) or delivered by mail.


Electronic W-2s

To set up your electronic W-2:

  • Sign in to CIS and select the Employee page
  • Select the Payroll, Taxes and Salary tile
  • Select W-2/W-2c Consent Forms. You will have the option to select to receive your W-2 electronically [view visual instructions]


Paper W-2s

For employees choosing to receive paper W-2 forms delivered by mail, the Home Address listed in CIS will be used for mailing the paper W-2 forms. It is very important that this is the current address for delivery of the 2018 W-2 forms in January 2019. If the Home Address is not correct, please change the address using the instructions below. Changing this address after January 14, 2019 may result in an undeliverable or lost W-2 form. A request to reprint W-2 forms will be charged a $12 processing fee.

To check or edit the Home Address for paper W-2 mail delivery:

  • Sign in to CIS and select the Employee page
  • Select the Change Bio/Demo Information tile
  • Select the Change Home/Mailing Addresses button and enter correct address


Former employees

If you are no longer employed at the university, your W-2 will be mailed at the end of January, unless you elected to receive your W-2 electronically. If you did not receive it in the mail or elected to receive it electronically, see the instructions above for electronic W-2s. Contact the UIT Help Desk at 801-581-4000 option 1 if you have issues logging in to CIS.
